Nondiscrimination policy, language assistance, and how to file a civil rights grievance.

Corizon Health complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability or sex (including pregnancy, sexual orientation and gender identity). We do not exclude people or treat them differently because of these characteristics.

Services we provide

Corizon Health:

  • Provides free aids and services to people with disabilities to communicate effectively with us, such as qualified sign language interpreters and written information in other formats, including large print and accessible electronic formats.
  • Provides free language services to people whose primary language is not English, such as qualified interpreters and information written in other languages.

If you need these services, ask the health services staff at the facility or contact us using the information below.

Filing a grievance

If you believe Corizon Health has failed to provide these services or discriminated in another way, you can file a grievance in person, by mail or through the Contact page. Address written grievances to the Civil Rights Coordinator, Corizon Health. If you need help filing a grievance, our staff will assist you. Filing a grievance will not affect the care you receive.

Filing with the Office for Civil Rights

You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically at ocrportal.hhs.gov, or by mail at 200 Independence Avenue SW, Room 509F, HHH Building, Washington, D.C. 20201, or by phone at 1-800-368-1019 (TDD 1-800-537-7697).
